Welcome to the fifth Annual Keystone Healthy Zone Schools Recognition Program. Completing this simple online assessment offers a first step in demonstrating your school's commitment to improving the health, well-being, and academic performance of your students.
There are 36+ questions about school policies and practices included in this assessment. We recommend that you print the sample assessment, talk with the school principal to designate someone to coordinate the responses, and submit the online assessment once you have gathered the information. Each school building can only submit one assessment.
PLEASE NOTE: All information will be confidential. Completion of the online assessment is time sensitive between March 3rd and April 18thApril 25th. Only schools who complete the online assessment will be recognized as Keystone Healthy Zone Schools. Keystone Healthy Zone Schools will receive a welcome packet in the mail in May 2008.
